Selective Clamping of the Segmental Artery during Robotic Partial Nephrectomy Surgery
Clamping of the main artery of the kidney results in ischemia (restriction of blood supply) across the entire kidney tissue. With the partial nephrectomy operation, there is a known relationship between the duration of occlusion of the renal artery and the loss of renal function. The longer the duration of occlusion of the renal artery is, the greater the loss of renal function is.
By clamping only the artery that supplies blood to the tumor (segmental artery) instead of the main artery that supplies the kidney, the renal function of the entire kidney tissue is maintained.
This video shows a robotic partial nephrectomy operation performed by selective clamping of the segmental artery supplying blood to the tumor located on the upper side of the kidney.
Minimal Margin Enucleoresection in Robotic Partial Nephrectomy
In partial nephrectomy surgery, the tumor tissue in the kidney is removed along with the healthy kidney tissue surrounding it. This is oncologically intended to leave no tumorous tissue in the kidney. However, the greater the size of the healthy tissue removed along with the tumor is, the greater the loss of renal function is. Therefore, it is aimed to remove as little healthy kidney tissue as possible without leaving any tumorous tissue behind. This technique applied in Robotic Partial nephrectomy is called MINIMAL MARGIN ENUCLEORESECTION.
This video shows a Robotic Partial Nephrectomy operation during which minimal amount of healthy kidney tissue is removed along with the kidney tumor.
Zero Ischemia in Robotic Partial Nephrectomy
Occlusion of the arteries that supply blood to the kidney with the aim of controlling bleeding during resection of the tumor is a common application in partial nephrectomy; as a result of the occlusion, the blood supply to the kidney is stopped. Every minute spent on removing the tumorous mass results in greater loss of renal function.
As a result of the radiological imaging procedures, partial nephrectomy operation can be performed on kidney tumors that are small, located on the outer surface of the kidney and do not involve the large arterial structures without occluding the kidney arteries. As a result of this procedure, which is called “ZERO-ISCHEMIA PARTIAL NEPHRECTOMY”, the renal function is completely maintained.
